Dear Brothers and Sisters:

This Office recently met with the "new" Workers Compensation and Disability Manager, Mark Morgan, concerning the numerous issues associated with Workers Compensation, Long Term Disability (LTD), Family Medical Leave Act (FMLA) and the Plan administrator, Liberty Mutual.

In our meeting Mr. Morgan committed to addressing and/or correcting the issues brought forward in an effort to ensure quality and consistency of the benefits and services being delivered to you as employees. In an effort to begin this task, Mr. Morgan has developed two different surveys which will be mailed to the employees who have either experienced a work related lost time injury within the last nine months or who have applied for LTD or FMLA.

The System Committee has reviewed both of the surveys and approved them for use with Bargaining Unit Employees. The surveys are completely confidential and will be mailed directly to the employee home address. We are asking for your help and vital input in this process should you receive the survey, we encourage you to fill it out and return it to Mr. Morgan as promptly as possible.
